老年人全髋关节置换术后假体周围骨折的处理

目的：总结人工髋关节置换术后股骨假体周围骨折治疗的经验和体会。方法：2005年1月至2011年1月治疗人工髋关节置换术后股骨假体周围骨折的患者11例，9例患者采用手术治疗，按照Vancouver分型标准进行分类，选择不同的手术方式进行治疗，包括：锁定加压接骨板固定，钢缆固定，同时结合异体骨板固定，关节翻修术等。结果：术后随访2-6年，平均5年，所有患者骨折均获得骨性愈合，骨折平均愈合时间为6个月（5-9个月），在最后随访时均有比较好的髋关节功能，Harris评分平均85分（77-93分）。结论：按照股骨假体周围骨折的类型，选用合适的治疗方法，全面设计术前计划，均可以取得比较满意的疗效。

Treatment of femur periprothetic fracture after hip arthroplasty

Objective To explore the treatment of femur periprothetie fracture after hip arthroplasty. Methods from January 2005 to January 2011, 11 patients with femur periprothetic fracture after hip arthroplasty were retrospectively analyzed. Of them, 9 patients were treated with operation. The surgery included open reduction and fixation with plate and with cerclage wires fixation or with allograft and revision. Results All patients were followed up for 2 to 6 years with average of 5 years. Bony heal was obtained in all patients. Av- erage Harris Score after the surgery was 85 （ 77 to 93 ）. Conclusions The detailed operation plan and proper treatment can get satisfac- tory outcomes.
