复方利多卡因乳膏涂抹喉罩在全身麻醉腹腔镜胆囊摘除手术中的应用效果

目的观察复方利多卡因乳膏涂抹喉罩全身麻醉下腹腔镜胆囊摘除术中抑制心血管不良反应和咽反射及术后咽痛的效果。方法选取择期行腹腔镜胆囊摘除术的患者70例，随机分为复方利多卡因乳膏喉罩组（A组）和石蜡油喉罩组（B组）。监测并记录两组的术前1d（T0）、人室时（T1）、喉罩插入时（T2）、喉罩插入后1—2min（T3）、呼之能睁眼时（T4）、拔出喉罩1rain时（T5）SBP、DBP、HR以及术后咽痛或咽部不适的情况。结果A、B组的SBP、DBP、HRT，比rrn均增加（P〈0．05）；To、T1、T2两组的SBP、DBP、HR、SP02比较，差异无统计学意义（P〉0．05）；B组T1、T4、T5时，SBP、DBP明显升高，HR明显增快（P〈0．05）。A组患者术中呛咳和术后咽痛、咽部不适感的发生率明显低于B组（P〈O．05）。结论复方利多卡因乳膏涂抹喉罩全身麻醉能有效抑制腹腔镜胆囊摘除术中心血管不良反应．能抑制咽反射和减少术后咽痛。

The application effect of compound lidocaine cream daubed on laryngeal mask in general anesthesia for laparoscopic cholecystectomy

Objective To observe the effect of compound lidocaine cream daubed in laryngeal mask for inhibiting of cardiovascular adverse reaction and gag reflex postoperative sore throat in general anesthesia for laparoscopic chole- cystectomy. Methods 70 patients with elective laparoscopic cholecystectomy were randomly divided into compound li- docaine cream laryngeal mask group （the group A） and paraffin oil laryngeal mask group （the group B）.SBP,DBP,HR before 1 day operation （T0）,when entering the operating room （T1）,when laryngeal mask inserting （T2）,after laryngeal mask inserting 1-2 min （T3）,when calling to open your eyes （T4） and when pulling out the laryngeal mask airwayl min （Ts） and sore throat or throat discomfort after operation were monitored and recorded. Results SBP,DBP, HR of the two groups at T1 increased compared to To （P〈0.O5）;there was no statistical difference of SBP,DBP,HR and SPO2 at T0,T1 and T2 of the two groups （P〉0.05）;SBP, DBP increased obviously and HR increased obviously at T3,T4 and T5 in the group B （P〈0.05）.The incidence rate of bucking durning operation,postoperative sore throat and throat discomfort after operation of patients in the group A were lower than that in the group B （P〈0.05）. Conclusion The compound lidocaine cream daubed on laryngeal mask for anesthesia can effectively inhibit cardiovascular adverse reaction and gag reflex, reducing postoperative sore throat in laparoscopic cholecystectomy.
