食管癌三野淋巴结清扫术术后复发特点及相关因素分析

目的探讨食管癌三野淋巴结清扫术术后肿瘤复发特点及其相关因素。方法回顾性分析2002年1月-2007年12月接受食管癌三野淋巴结清扫术治疗的184例胸中段食管鳞癌患者的临床资料,Logistic回归分析判定术后3年内肿瘤复发的危险因素。结果 3年内86例（46.7%）患者肿瘤复发,平均复发时间为11.4个月,其中局部-区域性复发46例（25.0%）、血行转移40例（21.7%）。单因素分析显示,食管癌术后复发与浸润深度、淋巴结转移、血管浸润、淋巴管浸润、淋巴结远处转移显著相关（P〈0.05）。Logistic分析显示,影响食管癌患者术后局部复发的因素为淋巴结转移、淋巴管浸润;影响术后血行转移复发的独立因素为血管浸润。结论食管癌的局部复发与淋巴结转移有关,而血行转移与血管浸润有关;食管癌三野淋巴结清扫术后可以降低局部复发率。
