上消化道出血原因分析及胃镜下治疗方法探讨

目的探讨上消化道出血原因及胃镜下治疗方法。方法将100例上消化道出血患者随机分为治疗组57例和对照组43例。治疗组针对不同病因,通过胃镜下给予喷洒药物、注射止血、金属钛止血和电凝止血治疗;对照组给予奥美拉唑口服、垂体加压素静脉滴注和对症治疗等药物治疗。比较2组疗效、出血情况及并发症。结果治疗组总有效率为96.49%高于对照组的62.79%,差异有统计学意义(P<0.01)。治疗组止血后再出血转外科手术治疗2例;对照组16例,其中出血后转胃镜治疗并止血成功7例,行外科急诊手术治疗8例,因出血过多死亡1例。胃镜治疗患者均未发生胃穿孔、窒息等并发症。结论胃镜治疗上消化道出血,可较好确定病因和发病部位,是上消化道出血较有效的非手术治疗方法。

Upper gastrointestinal bleeding cause analysis and gastroscope treatment mothed investigation

Objective To discuss the cause of upper gastrointestinal bleeding and gastroscope treatment mothed.Methods 100 cases patients with upper gastrointestinal bleeding were randomly divided into treatment group（57 cases） and control group（43 cases）.Treatment group according to different cause,through the gastroscope to spraying,injection hemostasis,metal titanium hemostatic and electrocoagulation;Control group were given omeprazole oral,intravenous vasopressin and symptomatic treatment.Compared the effect,bleeding situation and complication of two groups.Results The total effective rate of treatment group（96.49%） was higher than that of control group（62.79%）,the difference was statistically significant（P〈0.01）.Treatment group appeared turn the surgical to stop bleeding after bleeding two cases,the bleeding to stop bleeding after bleeding in 16 cases of control group,including hemorrhage and later the gastroscope treatment and seven cases of successful hemostasis,underwent a surgical emergency surgery in the treatment of eight cases,one cases of excessive bleeding to death.Gastroscopy the treatment of patients with no occurrence of gastric perforation,asphyxiation and other complication.Conclusion The gastroscopy treatment of upper gastrointestinal bleeding,can better determine the etiology and pathogenesis region of upper gastrointestinal bleeding is a more effective non-surgical treatment.
