前列腺突入膀胱的前列腺增生患者尿动力学分析及意义

摘    要:目的 探讨良性前列腺增生(benign-prostatic hyperplasia,BPH)突入膀胱的患者尿动力学特点.方法 选取302例在我院就诊并被确诊为BPH患者的尿动力学资料,根据前列腺增生突入膀胱部分长度(IPP),将全部患者分为A组和B组.A组:IPP≥1.0 cm组,共136例;B组:<1.0 cm,共166例.对比分析2组的检查结果.结果 A组中膀胱出口梗阻(94.1%)、逼尿肌不稳定(55.9%)、低顺应性发生率(41.2%)明显高于B组(75.3%、30.7%、16.9%),A组中最大尿流率[(5.02±1.76) ml/s]明显低于B组[(8.36 ±2.54) ml/s].结论 前列腺增生突入膀胱的患者膀胱出口梗阻(BOO)、道尿肌不稳定(DI)、低顺应性膀胱明显,应尽早实施手术治疗.

Abstract:Objective To investigate the urodynamic features of benign prostatic hyperplasia (BPH) with intravesical prostatic protrusion (IPP).Methods Urodynamic data of 302 patients that sought medical care in the hospital and were confirmed as BPH were selected for the study.In accordance with the length of IPP,the patients were divided into group A (136 cases) and group B (166 cases).The IPP length of group A was ≥ 1.0 cm,and the IPP length of group B was < 1.0 cm.Detection results were compared between the 2 groups.Results Rates of bladder outlet obstruction (94.1%),detrusor instability (55.9%) and lower compliance bladder(41.2%)were all significantly higher than those of group B(75.3% 、30.7% 、16.9%),and maximum uroflow rate of group A was [(5.02 ± 1.76) ml/s],which was significantly lower than that of group B [(8.36 ± 2.54) ml/s].Conclusion Bladder outlet obstruction,detrusor instability and lower compliance bladder in patients with IPP were prominent,which might require surgical treatment at an early date.
