改良一孔法微型腹腔镜治疗小儿腹股沟斜疝

目的探讨改良一孔法微型腹腔镜手术治疗小儿腹股沟斜疝的应用价值。方法采用特制穿刺针腹腔镜下一孔法巧妙手术步骤治疗小儿腹股沟斜疝40例,其中右侧22例,左侧10例,双侧8例。术前诊断单侧斜疝中有6例存在对侧隐性疝,均在腹腔镜下行内环口高位缝扎术。结果手术时间单侧疝平均12 min,双侧疝20 min。术后平均2 d出院。皮肤小戳口用皮肤胶粘合。10例有阴囊积气、腹膜外小血肿,1例复发,无其他严重并发症。结论改良一孔法微型腹腔镜下小儿腹股沟斜疝疝囊内环口高位缝扎术安全可靠、创伤小、美观且操作简便,术后恢复快,值得临床推广应用。

Modified one port minilaparoscopic treatment of indirect inguinal hernia in children

Objective To explore the value of modified one port minilaparoscopic treatment of indirect inguinal hernia in children.Methods Forty children with indirect inguinal hernia（right side in 22 cases,left side in 10 cases,bilateral in 8 cases） were treated by one port minilaparoscopic surgery with special made needle and intelligent surgical way.Preoperative diagnosis of unilateral indirect inguinal hernia,6 patients presented with contralateral hidden hernia,and were underwent high ligation of internal annulus under laparoscope.Results The mean of operative time was 12 mins in unilateral hernia and 20 mins in bilateral hernia.The average postoperative hospital stay was 2 days.The small cut was sticked with glue.10 cases had scrotal product gas,extraperitoneal small hematoma,1 case of recurrence,and no other serious complications.Conclusions Modified one port minilaparoscopic method in chidren with high ligation of internal annulus is safe,effective,minimally invasive,simple and rapid recovery.It is worthy of clinical application.
