心理干预对腹股沟疝修补术患者术前负性心理影响的临床研究

目的探讨术前心理干预对腹股沟疝修补术患者焦虑、抑郁的影响,以积累经验,指导临床工作。方法选取我院2007年1月至2009年3月择期行腹股沟疝修补手术的患者,共250例,分为观察组与对照组,观察组共125例,术前对其进行以认知行为疗法为主的心理干预;对照组共125例,术前只进行常规的护理。应用焦虑自评量表、抑郁自评量表评价心理干预前、后患者焦虑和抑郁评分的差别。结果观察组患者在心理干预后焦虑及抑郁评分明显低于对照组,观察组患者心理干预的效果与患者性别密切相关。结论以认知行为疗法为主的心理干预能明显缓解腹股沟疝修补术患者术前的负性情绪,尤其对女性患者临床效果更好,可以在临床中积极应用。

Clinical study of the effect of preoperative psychological intervention on negative psychology of patients with inguinal hernia

Objective To investigate the effect of preoperative psychological intervention to anxiety and depression of patients with inguinal hernia. Methods Two hundred and fifty patients with inguinal hernia, who were prepared to accept herniorrhaphy from January 2007 to March 2009 in our hospital, were randomly divided into observation group （n = 125 ） and control group （n = 125 ）. Patients in observation group treated mainly with cognitive behavior therapy and those in control group treated only with routine nursing. All patients were evaluated by Serf-rating Anxiety Scale （SAS） and Self- rating Depressive Scale （SDS） be- fore and after psychological intervention. Results The scores of SAS and SDS in observation group were significantly higher than those in control group, and the effect of psychological intervention in observation group was closely related to gender. Conclusions The results suggested that the preoperative psychological intervention mainly with cognitive behavior therapy could obviously relieve negative emotions of patients with inguinal hernia, especially to female patients.
