老年胆结石患者手术治疗方法和护理干预及其效果观察

目的观察老年胆结石患者手术治疗方法、护理干预及其效果。方法回顾性分析2018年1月至2020年1月本院收治的100例老年胆结石患者的临床资料,依据手术治疗方法、护理干预方法的不同分为小切口胆囊切除术配合常规护理干预组(对照组,n=50)与腹腔镜胆囊切除术配合综合护理干预组(研究组,n=50)。比较两组临床疗效、手术相关指标、止痛药使用情况、术后并发症发生率。结果研究组治疗总有效率为98.0%,高于对照组的70.0%(P<0.05)。研究组术中出血量显著少于对照组(P<0.05),手术时间、排气时间、进食时间、住院时间均显著短于对照组(P<0.05)。研究组止痛药使用率、术后并发症发生率分别为16.0%、4.0%,均显著低于对照组的36.0%、20.0%(P<0.05)。结论老年胆结石患者腹腔镜胆囊切除术配合综合护理干预的效果较小切口胆囊切除术配合常规护理干预好,值得推广。

Surgical treatment,nursing intervention and effects observation of elderly patients with gallstone

Objective To observe the surgical treatment,nursing intervention and effects of elderly patients with gallstones.Methods 100 cases of elderly patients with gallstones in our hospital from January 2018 to January 2020 were retrospectively analyzed,and they were divided into small incision cholecystectomy combined with routine nursing intervention group(control group,n=50)and laparoscopic cholecystectomy combined with comprehensive nursing intervention group(study group,n=50)according to the surgical treatment and nursing intervention methods.The clinical efficacy,operation related indicators,analgesic uses and postoperative complications between the two groups were compared.Results The total effective rate of the study group was 98.0%,which was significantly higher than 70.0%of the control group(P<0.05).The intraoperative blood loss of the study group was significantly less than that of control group(P<0.05),and the operation time,exhaust time,eating time,hospitalization time were significantly shorter than the control group(P<0.05).The use rate of painkillers and incidence of postoperative complications in the study group was 16.0%and 4.0%,which were significantly lower than the control group(36.0%and 20.0%)(P<0.05).Conclusion The effect of laparoscopic cholecystectomy combined with comprehensive nursing intervention in elderly patients with gallstone is better than small incision cholecystectomy combined with routine nursing intervention.It is worthy of clinical promotion.
