乙酰半胱氨酸雾化治疗小儿支气管肺炎的疗效及对血清炎性因子和肺功能的影响

摘    要:目的探讨乙酰半胱氨酸雾化治疗小儿支气管肺炎的疗效及对血清炎性因子和肺功能的影响。方法选取本院2018年6月至2019年5月收治的儿童支气管肺炎患儿100例,随机分为对照组和观察组,各50例。两组患儿均给予吸氧、抗感染等对症治疗,对照组在此基础上给予布地奈德混悬液治疗,观察组在对照组基础上给予乙酰半胱氨酸溶液治疗。比较两组临床疗效及治疗前后血清炎性因子(CRP、PCT及TNF-α)和肺功能指标(FVC、FEV1及FEV1%)。结果观察组总有效率为96.00%,明显高于对照组的80.00%,差异有统计学意义(P<0.05);治疗前,两组CRP、PCT、TNF-α、FVC、FEV1及FEV1%比较差异无统计学意义;治疗后,两组CRP、PCT和TNF-α均较治疗前降低,FVC、FEV1和FEV1%均升高,差异有统计学意义(P<0.05),且观察组治疗后CRP、PCT、TNF-α均低于对照组,FVC、FEV1及FEV1%均高于对照组,差异有统计学意义(P<0.05)。结论乙酰半胱氨酸雾化治疗可显著改善小儿支气管炎患儿临床症状,降低血清CRP、PCT及TNF-α炎性因子水平,提高肺功能,疗效显著,值得临床推广应用。

Abstract:Objective To investigate the effect of atomization of acetylcysteine on bronchopneumonia in children and its effect on serum inflammatory factors and lung function.Methods 100 children with bronchopneumonia admitted to our hospital from June 2018 to May 2019 were randomly divided into control group and observation group,with 50 cases in each group.Both groups of children were given oxygen,anti infection and other symptomatic treatment,the control group was treated with budesonide suspension on this basis,and the observation group was treated with acetylcysteine solution on the basis of the control group.The effect,changes of serum inflammatory factors(CRP,PCT and TNF-α)and lung function indexes(FVC,FEV1 and FEV1%)were compared before and after treatment between two groups.Results The total effective rate of the observation group was 96.00%,which was higher than 80%in the control group,the difference was statistically significant(P<0.05).Before treatment,there was no significant difference in CRP,PCT,TNF-α,FVC,FEV1 and FEV1%between the two groups.After treatment,CRP,PCT,TNF-αdecreased,FVC,FEV1 and FEV1%increased in the two groups compared to before treatment the difference was statistically significant(P<0.05).After treatment,CRP,PCT,TNF-αin the observation group were lower than those in the control group,and FVC,FEV1 and FEV1%were higher in the control group(P<0.05).Conclusion The atomization of acetylcysteine can significantly improve the clinical symptoms of children with bronchitis,reduce the levels of serum CRP,PCT and TNF-αinflammatory factors,improve the lung function,and the effect is effective.It is worthy of clinical promotion and application.
