孕产妇抗磷脂抗体水平的检测及意义

目的探讨孕产妇血抗磷脂抗体水平检测及其对妊娠结局的影响。方法选取2016年7月至2018年11月于本院孕检及分娩的有病理性妊娠史的孕产妇708例作为病例组,另外选取同期于本院进行孕检和分娩、无血栓史、无免疫疾病史的正常孕产妇602名为正常组,比较两组APLs阳性率及妊娠结局(流产、早产、新生儿窒息等)。结果病例组单纯LA、ACL、β2GPⅠ阳性率及任意2项或3项阳性率、APLs总阳性率均明显高于正常组(P<0.05),病例组中APLs阳性患者早产发生率明显高于阴性患者(P<0.05)。正常组中,APLs阳性患者早产、新生儿窒息发生率明显低于阴性患者(P<0.05)。结论APLs在病理妊娠过程中发挥重要作用,其阳性可一定程度影响妊娠结局,临床工作中对于有病理妊娠史的孕产妇,应尽早进行APLs检测,及时采取干预措施,尽量改善病理妊娠状态,改善妊娠结局。

Detection and significance of antiphospholipid antibody in pregnant women

Objective To investigate the level of serum antiphospholipid antibody in pregnant women and its influence on pregnancy outcome.Methods From July 2016 to November 2018,708 pregnant women with pathological pregnancy history who underwent pregnancy examination and delivery in our hospital were selected as the case group,and 602 normal pregnant women with no history of thrombosis and immune disease were selected as the normal group.The positive rate of APLs and pregnancy outcome(abortion,premature birth,neonatal asphyxia,etc.)in the two groups were recorded.Results The positive rates of LA,ACL,β2GPⅠ,any two or three items,and the total positive rate of APLs in the case group were significantly higher than those in the normal group(P<0.05).The incidence of preterm birth in patients with APLS positive was significantly higher than that in patients with negative APLS(P<0.05).In the normal group,the incidence of premature delivery and neonatal asphyxia in APLS positive patients was significantly lower than that in negative patients(P<0.05).Conclusion APLS plays an important role in pathological pregnancy,and its positive can affect the pregnancy outcome to a certain extent.In clinical work,for pregnant women with pathological pregnancy history,we should detect APLS as soon as possible and take timely intervention measures to improve the pathological pregnancy status and pregnancy outcome.
