不同时期发热待查的病因分布及临床特征研究

目的探讨不同时期、年龄及热程发热待查(FUO)患者的病因分布及临床特征。方法选取蚌埠医学院第一附属医院感染科2008年1月至2018年1月收治的540例FUO患者的临床资料。统计患者的病因分布及临床特征,比较2008年至2012年与2013年至2018年、年龄<60岁与≥60岁、热程<3个月与≥3个月患者病因构成情况。结果540例患者中感染性疾病357例(66.11%),结蹄组织疾病50例(9.26%),肿瘤性疾病41例(7.59%),其他疾病92例(17.04%);不同时期FUO患者病因构成比例比较差异无统计学意义;年龄<60岁FUO患者感染性疾病、结蹄组织疾病及其他疾病等病因构成比均高于年龄≥60岁患者(P<0.05);热程<3个月FUO患者感染性疾病及其他疾病等病因构成比均高于热程≥3个月患者,结蹄组织疾病病因构成比小于热程≥3个月患者(P<0.05)。结论感染性疾病为FUO的主要病因以咳嗽为主要特征表现,结蹄组织疾病、肿瘤性疾病及其他疾病为FUO的次要病因,临床通过了解FUO的病因及特征分布,可为FUO的临床诊断与治疗提供指导依据。

Study on the etiological distribution and clinical characteristics of fever to be examined (FUO) in different periods

Objective To investigate the distribution and clinical characteristics of etiology in FUO patients at different stages,ages and stages.Methods The clinical data of 540 FUO patients admitted to the infection department of the First Affiliated Hospital of Bengbu Medical College from January 2008 to January 2018 were selected.The etiology distribution and clinical characteristics of the patients were statistically analyzed,and the etiology composition of the patients in 2008-2012 and 2013-2018,age<60 years and≥60 years,heat stroke<3 months and≥3 months were compared.Results Among the 540 patients,357 cases(66.11%)had infectious diseases,50 cases(9.26%)had clubfoot tissue diseases,41 cases(7.59%)had tumor diseases and 92 cases(17.04%)had other diseases.There was no significant difference in the proportion of FUO patients in different periods.The proportion of FUO patients with infectious diseases,clubfoot tissue diseases and other diseases was high In patients with age≥60 years old(P<0.05),the proportion of FUO patients with fever<3 months old was higher than that of FUO patients with fever≥3 months old(P<0.05).Conclusion Infectious diseases are the main cause of FUO,cough is the main characteristic manifestation,and hoof tissue disease,tumor disease and other diseases are the secondary cause of FUO.Understanding the etiology and characteristic distribution of FUO can provide guidance for the clinical diagnosis and treatment of FUO.
