缺血性心肌病与扩张型心肌病的超声心动图特征比较与鉴别诊断

目的分析超声心动图在心肌病患者中的应用效果及临床价值。方法选取2018年7月至2019年5月本院收治的因射血分数低、心功能不全,且超声检查提示心脏扩张的100例患者为研究对象,所有患者实施冠状动脉造影后,结合临床综合诊断,明确缺血性心肌病者35例,扩张型心肌病者40例,酒精性心肌病16例,克山病9例。结果经分析,两组患者右室内径、左房内径、左室舒张末内径、二尖瓣舒张晚期血流速度峰值、主动脉瓣口收缩期峰值速度、左室舒末容积、左室缩末容积、射血分数比较差异具有统计学意义(P<0.05)。结论超声心动图能够准确鉴别缺血性心肌病及扩张型心肌病,可以为临床诊断以及治疗该疾病提供科学依据,值得临床推广。

Comparison of echocardiographic features and differential diagnosis between ischemic cardiomyopathy and dilated cardiomyopathy

Objective To analyze the application effect and clinical value of echocardiography in patients with cardiomyopathy.Methods A total of 100 patients with low ejection fraction,cardiac insufficiency,and ultrasound examination suggesting cardiac dilatation in our hospital from July 2018 to May 2019 were selected as the research subjects.All patients underwent coronary angiography,combined with comprehensive clinical diagnosis,identified 35 cases of ischemic cardiomyopathy,40 cases of dilated cardiomyopathy,16 cases of alcoholic cardiomyopathy,9 cases of Keshan disease.Results After analysis,the right ventricle diameter,left atrial inner diameter,left ventricular end diastolic diameter,peak mitral valve late diastolic blood flow velocity,aortic valve systolic peak velocity,left ventricular end diastolic volume,and left ventricular contraction There was signficant difference between the final volume and ejection fraction(P<0.05).Conclusion Echocardiography can accurately identify ischemic cardiomyopathy and dilated cardiomyopathy.It can provide scientific basis for clinical diagnosis and treatment of this disease,and it is worth popularizing.
