腹腔镜根治性前列腺切除术后尿控恢复影响因素的分析

目的研究分析腹腔镜根治前列腺切除术后影响尿控恢复的因素。方法选取2013年5月至2018年6月本院接收的行腹腔镜根治性前列腺切除术者124例为研究对象,根据患者术后恢复情况分为未恢复组(n=82)和恢复组(n=42)。采用单因素、多因素分析影响患者术后尿控恢复的因素。结果恢复组患者FPL指标长于研究组,保留神经血管束率、后方重建率和膀胱颈水平≥1级比例均高于未恢复组(P<0.05);术后1个月,尿控恢复预后作为因变量,多因素Logistic回归分析表明,FPL和保留神经血管束率、后方重建率和膀胱颈水平≥1级比例为影响尿控恢复的影响因素(P<0.05)。结论影响腹腔镜根治性前列腺切除术后尿控恢复的因素包括后方筋膜重建和术后颈水平、FPL和神经血管束保留。

Analysis of factors influencing urinary control recovery after laparoscopic radical prostatectomy

Objective To study the factors affecting the recovery of urinary control after laparoscopic radical prostatectomy.Methods 124 cases patients undergoing laparoscopic radical prostatectomy from May 2013 to June 2018 were selected as study subjects in our hospital.They were assigned to the unrecovered group(n=82)and the recovery group(n=42)according to the postoperative recovery.Single factor and multivariate analysis were used to influence the factors of postoperative urinary control recovery in patients.Results The FPL index of the recovery group was longer than that of the unrecovered group.The retention of neurovascular bundle rate,posterior reconstruction rate,and bladder≥1 grade were higher than those of the unrecovered group(P<0.05).Prognosis as a dependent variable,multivariate logistic regression analysis showed that the ratio of FPL and retained neurovascular bundle rate,posterior reconstruction rate and bladder neck level≥1 were the influencing factors of urinary control recovery(P<0.05).Conclusion Factors affecting urinary management recovery after laparoscopic radical prostatectomy include posterior fascia reconstruction and postoperative neck level,FPL and neurovascular bundle retention.
