妊娠期亚临床甲减患者甲状腺激素治疗剂量调整的临床观察

目的探讨妊娠期亚临床甲减患者甲状腺激素治疗剂量调整的临床效果。方法选取2018年7月至2019年12月本院收治的妊娠期亚临床甲减患者120例作为研究对象,依据是否怀孕早期(8周之前)检查出来分为对照组和观察组,每组60例。两组均给予甲状腺激素治疗,比较两组不同孕期甲状腺激素剂量变化、各个孕期甲状腺激素TSH的水平情况。结果妊娠12、16、20、24、28~32、36、38~40周时,研究组所需甲状腺激素剂量显著低于观察组(P<0.05);观察组各个孕期的TSH水平均低于对照组,且于28周后显著降低(P<0.05)。结论对于孕妇妊娠亚临床甲减患者应予以及时补充甲状腺激素,需要的甲状腺激素剂量在妊娠28周后增加缓慢,临床医生需合理进行调节。

Clinical observation on dose adjustment of thyroid hormone therapy in subclinical hypothyroidism patients during pregnancy

Objective To explore the clinical efficacy of adjusting the dose of thyroid hormone in the patients with subclinical hypothyroidism during pregnancy.Methods 120 cases of subclinical hypothyroidism in pregnancy treated from July 2018 to December 2019 were selected as the reseach subjects,and divided into control group and observation group according to the early pregnancy(before 8 weeks),with 60 cases in each group.They were given thyroid hormone for treatment,compare the changes of thyroid hormone doses in different pregnancy periods and the levels of thyroid hormone TSH in each pregnancy period between the two groups.Results At 12,16,20,24,28 to 32,36,38 to 40 weeks of pregnancy,the dose of thyroid hormone required in the study group was significantly lower than that of the observation group(P<0.05);the TSH levels of the observation group during each pregnancy were lower than the control group,and significantly decreased after 28 weeks(P<0.05).Conclusion Thyroid hormone should be supplemented to pregnant women with subclinical hypothyroidism in time.The dose of thyroid hormone needed increases slowly after the seventh month of pregnancy.Compared with the patients with subclinical hypothyroidism during pregnancy,the dose of thyroid hormone is quite small,and clinical doctors need to adjust it reasonably.
