经肝动脉化疗栓塞治疗肝癌破裂出血23例

摘    要:目的评价经肝动脉化疗栓塞(TACE)在肝癌破裂出血抢救治疗中的作用.方法回顾性分析1995-2000年进行TACE治疗的23例肝癌破裂出血临床资料.结果血管造影找到出血部位者有18例(78.3%),未找到出血部位者有5例,其中1例推注碘油后找到出血部位.TACE有效控制出血者有20例(87.0%),其中5例于1~2个月内死于肝功能及全身衰竭,平均生存8.1个月,生存1年以上者8例,最长者3年7个月,其余3例止血不满意者于24h内死于失血性休克.结论TACE是治疗肝癌破裂出血的有效手段,尤其对不能外科止血治疗者至关重要.

Abstract:Objective\ To evaluate the role of transcatheter arterial chemoembolization (TACE) for patients with spontaneous rupture of hepatoma. Methods\ Analysis the clinical data of TACE treatment in twenty-three patients with spontaneous rupture of hepatoma from 1995 to 2000. Results\ By interventional angiography,eighteen cases (78.3%) had been found the bleeding places, but five cases hadn't. There were twenty-three patients received TACE treatment. Bleeding was completely controlled in twenty patients (87.0%), five of them died of liver or body failure from one to two months. The average survival time was 8.1 months. Eight of them had survived for over one year, and the longest survival time was three years and seven months. The rest three ones had not been stanched and died of shock within 1 months. Conclusion\ TACE is an effective method in the treatment of spontaneous rupture of hepatoma,especially important to the patients who can't accept surgical treatment.
