XELOX和OLF方案在晚期胃癌治疗中的疗效与毒副反应比较

目的探讨奥沙利铂联合希罗达方案（XELOX）治疗晚期胃癌的疗效和毒副反应。方法对我院2006年5月至2007年7月间采用XELOX化疗方案治疗的晚期胃癌进行回顾性分析，并与奥沙利铂、5氟脲嘧啶和醛氢叶酸联合化疗方案（OLF）进行比较。XELOX组41例，OLF组44例。结果XELOX方案组总有效率36．5％，疾病进展时间5．6个月，OLF方案组为35％和5．2个月，两组间无显著性差异；主要的毒副反应腹泻、恶心、呕吐、骨髓抑制和肝功能损害两组间无显著性差异，XELOX方案组的手足综合征发生率明显高于OLF方案组（P〈0．01），但都为Ⅰ～Ⅱ度反应。结论XELOX和OLF方案治疗晚期胃癌的疗效相近，毒副反应可耐受，但XELOX方案用药更方便，且在复治患者中有一定的优势。

Comparison of combination chemotherapy with XELOX and OLF in the treatment of advanced gastric cancer

Objective To study the efficacy and toxicity of oxplatin combined with xeloda in the treatment of advanced gastric cancer. Methods A retrospective analysis in the treatment of advanced gastric cancer with XELOX from May 2005 to April 2007 was done, compared with the combination of oxplatin, 5-fu, leuccvorin （OLF）. 41 patients in XOLOX group and 44 patients in OLF group were analyzed. Results The effective rate and time to disease progression （TFP） in XELOX group were 36.5% and 5.6 months, and in OLF group were 35% and 5.2 months respectively, there was no obvious difference between two groups. There were diarrhea, nausea, vomiting, myelosuppression and dysfunction of live in the side-effects of two groups. The incidence of hand-foot syndrome of XELOX was obviously higher than that of OLF, but most of them were slight （grade Ⅰ-Ⅱ ）, the other side-effects had no difference between two groups. Conclusion The effects in the treatment of advanced gastric cancer with XELOX regimen or OLF regimen are similar and the side-effects are slight, but XELOX regimen is more convenient and has invariable dominance in recurrent patients.
