血清降钙素原在危重患者细菌感染检测中的应用

目的评价血清降钙素原（PCT）对危重患者细菌感染诊断的临床价值。方法选择重症监护病房（intensivecareunit,ICU）中30例细菌感染的患者作为感染组,并以30例健康献血者作为对照组,应用免疫比浊法测定其血清PCT及CRP水平,同时作外周血白细胞（WBC）计数,结果作统计分析。结果感染组与对照组比较PCT浓度显著增高（P〈0.001）,CRP浓度显著增高（P〈0.005）,WBC水平显著升高（P〈0.001）;感染组PCT阳性率与血培养阳性率比较明显升高,差异有显著性（P〈0.005）,但所有血培养阳性的病例PCT均为阳性,符合率为100%;当感染得到控制后PCT浓度也随之下降。结论血清PCT是危重患者细菌感染诊断及临床治疗的一个特异性标志物。

Application of serum procalcitonin in critical patients with bacterial infection

Objective To evaluate the clinica1 value of serum procalcitonin（PCT） in the diagnosis of bacteria1 infections in the critical patients. Methods 30 patients in ICU were chosen as an infected group,their peripheral white blood cells were recorded and the PCT and CRP level in serum were detected by immunoturbidimetry. Another 30 healthy blood donors were chosen as a control group. Results The levels of PCT in bacterial infected group were significantly higher than that in control group （P〈0.001）,so as the levels of CRP（P〈0.005） and WBC（P〈0.001）. In bacterial infected group,the positive rate of PCT was higher than that of hemocultures（P〈0.005）,but the positive rate of PCT went up to 100% in the positive hemocultures. When the infection was under control,the PCT level decreased correspondingly. Conclusion Serum PCT is a specific marker in the diagnosis and therapy of bacterial infections in the critica1 patients.
