胃间质瘤32例临床诊治分析

目的探讨胃间质瘤的术前诊断方法和有效的治疗手段。方法回顾性分析我院收治的32例胃间质瘤患者的临床资料。结果胃间质瘤缺乏典型的临床表现,半数以上可有消化道出血,还可表现为腹痛、腹部不适及腹部包块。27例胃镜发现异常,28例腹部CT检查均发现异常。本组32例均经手术及术后病理证实,免疫组化检查CD117阳性30例（93.7%）,CD34阳性23例（71.9%）。手术切除后5例复发者二次手术治疗,2例行姑息性减瘤手术者于6月内死亡。结论电子胃镜、超声内镜、消化道钡餐透视、腹部CT检查有助于手术前诊断,免疫组织化学检查可确诊胃间质瘤,手术后应行定期检查。

Diagnosis and treatment of gastric stromal tumors

Objective To investigate preoperative diagnostic methods and effective treatment for gastric stromal tumors. Methods A retrospective review was performed on 32 patients with gastric stromal tumors （GST）.Results The patients with GST had no typical clinical findings,and were often accompanied with gastrointestinal bleeding,abdominal pain and abdominal mass,27 cases were found abnormities by gastroscopy.28 cases were found abnormities by abdominal computed tomography（CT）. All the patients were treated with operation, and were approved with pathology.Immunohistochemicalstaining showed CD117 positive in 30 cases （93.7%）,and CD34 positive in 23 cases （71.9%）.Two cases who underwent palliative resection died in 6 months,Conclusion Preoperative gastroscopic endoscopic ultrasound （EUS）,barium examinations and computed tomography （CT）can help diagnose.Immunohistochemical staining plays an important role in diagnosis,and regular examinations are necessary for the patients with GST.
