抑癌基因PTEN、血管内皮生长因子的表达在胃癌进展和预后中的相互关系

目的 评价胃癌组织中微血管密度（MVD）、血管内皮生长因子（VEGF）、抑癌基因PTEN的表达以及它们之间的相互关系,探讨它们与胃癌生物学行为及预后的关系,寻找判断预后和疗效的生物学指标。方法 用免疫组化法将我院1997年1月-2001年12月经病理诊断证实的胃癌手术标本进行微血管密度、VEGF、PTEN检测,并作临床随访和资料统计分析。结果 ①微血管密度、VEGF的表达与胃癌患者的性别、年龄无关;与肿瘤的大小、Borrmann分型、分化程度、浸润深度、淋巴结转移、肿瘤分期呈正相关（P〈0.05,P〈0.01）;与患者的术后生存时间呈负相关。②胃癌组织中PTEN的阳性表达率为46.7%,与患者的术后生存时间呈正相关。③PTEN的表达与VEGF的表达无显著相关性（P〉0.05）;VEGF和PTEN双因素组合,对胃癌患者预后有不同的影响。结论 MVD、VEGF、PTEN的表达与胃癌的生物学行为及患者的预后有密切关系;VEGF、PTEN的共同作用影响胃癌患者的预后。

Expression of PTEN and vascular endothelial growth factor （VEGF） correlates with biological character and prognosis of gastric cancer

Objective To evaluate relationship between microvessel density （MVD）, expression of endothelial cell growth factor （VEGF） and that of tumor suppressor gene PTEN; and their effect on tumor biological character and patient prognosis in gastric cancer. Methods Sixty surgical specimens of gastric cancer between 1997.1 to 2001.12 were selected CD34, VEGF, PTEN were determined by immunohistochemistry SABC method. Results ①MVD, expression of VEGF and PTEN have no correlation with gender, age of patients. MVD and expression of VEGF correlates significantly and positively with the size of the cancer （P〈0.05）, invasion depth （P〈0.05）, lymph node metastasis （P〈0.01）, stage of cancer （P〈0.01）, and correlates strongly and adversely with patient prognosis （P〈0.05） . ②In 46.7% gastric cancer PTEN was positive , others negative . There were significant and adverse correlations between PTEN and tumor size （P〈0.01）, Borrmann type （P〈0.05）, lymph node metastasis （P〈0.01）, tumor stage （P〈0.05）; that between PTEN and patient prognosis was positive （P〈0.01）. ③MVD,VEGF correlates significantly and positively each other （P〈0.05）; The significant correlation between PTEN and MVD was adverse（P〈0.05）; As to PTEN and VEGF, have no strong correlation （P〉0.05）. Conclusion ①MVD, VEGF, PTEN correlate with the biological characters of gastric cancer and patient prognosis.②There are interactions between VEGF and PTEN, which play important role in patient prognosis.
