原发性巨球蛋白血症临床病理观察

目的探讨原发性巨球蛋白血症（Waldenstrom macroglohulinemia）的骨髓病理和免疫组化特点。方法对1例原发性巨球蛋白血症患者于髂后上棘取骨髓活检标本，石蜡包埋、切片，常规HE染色及免疫组化染色，光镜观察。结果骨髓主要由形态成熟的小淋巴细胞和浆细胞样淋巴细胞浸润，正常造血细胞显著减少。免疫组化染色CD20弥漫阳性，K链（＋），入链（＋），CD5局灶阳性，CD3局灶阳性。结论原发性巨球蛋白血症属低度恶性淋巴浆细胞淋巴瘤，骨髓活检HE染色及免疫组化染色对该病的诊断和鉴别诊断具有重要意义。

Clinicopathological observation of waldenstrom macroglobulinemia

Objective To explore the bone marrow pathological and immunohistochemical features of Waldenstrom macroglobulinemia. Methods Bone marrow biopsy specimen was taken from posterior superior iliac spine from a patient with Waldenstrom macroglobulinemia,Paraffin-embedded sections stained with conventional HE and immunohistochemical methods were observed under optical microscope, Results The bone marrow was infiltrated with morphologically mature small lymphocytes and plasmacytoid lymphocytes.The normal hemopoietic cells in the bone marrow decreased significally,Immunohistoehemieal staining showed diffuse CD20 （＋）,kappa light chain （＋）,lambda light chain （＋）,focal CD5 （＋）,focal CD3 （＋）. Conclusion Waldenstrom macroglobulinemia is a low-grade lymphoplasmacytic lymphoma.HE and immunohistochemical staining of bone marrow biopsy specimen is of important significance for diagnosis and differential diagnosis,
