首次无痛胃镜检查病人并发症发生危险因素调查及其护理干预

目的:探讨首次无痛胃镜检查病人并发症发生危险因素,明确相应护理干预措施。方法:选择2017年1月—2019年8月收治的首次无痛胃镜检查病人140例为研究对象,借助回顾性分析法收集其临床资料,统计计算并发症发生率,通过单因素、多因素Logistic回归分析确定并发症发生危险因素,确定护理干预措施。结果:我院首次无痛胃镜检查病人并发症发生率为14.29%;单因素分析显示,影响无痛胃镜检查并发症发生因素有年龄、异常气道、自我效能水平低、心血管系统疾病史、禁食禁水时间、焦虑情绪(P<0.05);多因素Logistic回归分析显示,无痛胃镜检查并发症发生危险因素有异常气道、心血管系统疾病史、禁食禁水>3 h、焦虑自评量表(SAS)评分≥50分、自我效能水平低(P<0.05)。结论:首次无痛胃镜检查病人易发生相关并发症,且异常气道、心血管系统疾病史、禁食禁水>3 h、SAS评分≥50分、自我效能水平低为检查并发症发生危险因素,需结合危险因素开展护理工作,缓解病人焦虑情绪,提升其自我效能水平。

Investigation on risk factors of complications in patients undergoing painless gastroscopy for the first time and nursing intervention

Objective:To investigate the risk factors of complications in patients undergoing painless gastroscopy for the first time,and to clarify the corresponding nursing intervention measures.Methods:A total of 140 patients undergoing painless gastroscopy in our hospital from January 2017 to August 2019 were enrolled.The clinical data were collected by retrospective analysis,and the incidence of complications was statistically calculated.The risk factors for complications were determined by single-factor and multivariate logistic regression analysis,to determine the nursing intervention measures.Results:The complication rate of patients undergoing painless gastroscopy for the first time in our hospital was 14.29%;single-factor analysis showed that the influencing factors for painless gastroscopy complications included age,abnormal airway,low self-efficacy,history of cardiovascular system disease,fasting and water-free period,and anxiety(P<0.05);Multivariate logistic regression analysis showed that the risk factors for painless gastroscopy complications were abnormal airway,history of cardiovascular system disease,fasting and water-free period>3 h,scores of Self-rating Anxiety Scale(SAS)≥50 points,and low self-efficacy level(P<0.05).Conclusions:Patients undergoing painless gastroscopy for the first time were prone to related complications.Abnormal airways,the history of cardiovascular disease,fasting and water-free period for more than 3 hours,SAS≥50 points,and low self-efficacy were risk factors for complications.Nursing work should be carried out as necessary combined with risk factors to relieve patients′anxiety and improve their self-efficacy.
