多普勒组织成像技术对右心起搏器电除极状态的研究

摘    要:目的 应用DTI技术分析评价安置右心起搏器患者的异常电除极状态。方法:研究人群58例,分为三组,A组为20例近期安装VVI型或DDD型右心起搏器的患者;B组为18例心电图证实为完全性左束支传导阻滞的患者,C组则为正常心电图的健康人群20例。结果:1.DTA图显示:①除极起源:A组的心室除极源于右室心尖部,与C组有显著差异;②除极顺序:胸骨旁心腔心切面A组加速度变化顺序为右室前壁心尖-右室前壁中部、

Abstract:Objective: To demonstrate the ventricaular depolarization in patients with right ventricular pacing using Dopper tis- sue imaging. Methods: The studied population consisted of three groups: 20 patients in group A were right ventricular pace - maker; 18 patientsion group B were patients with complete left bundle branch block; in group C were 20 healthy volunteers. Results: The origin of abnormal depolarization in group A was located in the apex of right ventricular, The origin of ventricular depolarization in group C was located in IVS area, there was significant difference between group C and A (B); In parasternal left ventricular long axis view, the contraction of left ventricular posterior wall in group A and B were significantly slower than that in group C. Furthermore, the contraction of left ventricular posterior wall in group A was earlier than that in group B. The the intervals from the onset of Q wave on ECG to the beginning. the end of systolic band of left ventricular posterior wall in group A were shorter than that in group . Conclu- sion: DTI can be used to assess the abnormal ventricular depolarization in parients with pace - maker. It seems that the depolarization of left ventricular in right ventricular pace - maker is earlier than that in the patients with complete left bundle branch bloch.
