肝硬化患者动脉血氧变化78例分析

摘    要:目的:探讨肝硬化患者动脉血氧变化与临床的关系。方法:应用血气分析仪测定78 例肝硬化患者其动脉血氧分压(PaO2)及氧饱和度(SaO2),其中Child Pugh分级A级15例、B级35例、C级28例。选择同期正常人30例作对照,比较两组的PaO2、SaO2值。结果:肝硬化组平均动脉PaO2与正常对照组比较有极显著差异(P<0.01);SaO2 两组比较,有显著差异(P<0.05)。肝硬化患者不同门静脉内径、有无蜘蛛痣、不同肝功能分级的PaO2和SaO2值比较,均有极显著差异(P<0.01)。结论:肝硬化患者低氧血症发生率高,其严重程度与有无蜘蛛痣、门静脉内径有关;发生率与Child Pugh分级有关。

Abstract:Objective: To study the relationship between the arterial oxygen changes and its clinical characteristics in patients with liver cirrhosis. Methods: The partial pressure of the arterial oxygen (PaO_ 2) and oxygen saturation of arterial blood(SaO_ 2) in 78 cases with liver cirrhosis were examined by automatic blood gas analyzer. Of them,15 were classified as Child-Pugh A,while 35 Child-Pugh B and 28 Child-Pugh C. 30 healthy volunteers as controls were studied using the same methods. Results: Compared the studied group with the control group, PaO_ 2 fell significantly(P<0.01), the difference of SaO_ 2 was significant(P<0.05).Compared the cirrhosis patients with different portal vein bore,with or without nevus araneus or with different Child-Pugh, the difference in PaO_ 2 and SaO_ 2 were significant, respectively(All P<0.01). Conclusion:The incidence of hypoxemia was high in patients with liver cirrhosis. Severity of hypoxemia was related with portal vein inner diameter and spider angioma. The incidence of hypoxemia was related with Child-Pugh grade.
