达芬奇机器人系统在脾切除术中的应用

摘    要:目的:探讨达芬奇机器人手术系统应用于脾切除术的安全性与有效性。方法:回顾分析2012年2月至2013年2月为5例患者行机器人脾切除术的临床资料。5例手术均由同一术者完成。分析评估手术情况、术后并发症发生情况及术后恢复情况。结果:5例患者均顺利完成达芬奇机器人手术系统脾切除术,无一例中转开腹,术后未发生并发症。患者均恢复良好出院。手术时间平均(145±50)min,达芬奇机器人系统装配时间平均(10.5±5.2)min,术中出血量平均(45±12)ml,术后平均住院(3.3±2.5)d。结论:达芬奇机器人手术系统应用于脾切除术安全、可行,手术创伤小,术中出血少,疼痛轻,明显缩短了住院时间,具有良好的可操作性及实用性。

Abstract:Objective:To discuss the safety and effectiveness of robot-assisted splenectomy by Da Vinci system. Methods:The clinical data of 5 patients who underwent robot-assisted splenectomy by Da Vinci system in general surgery department of Nanjing General Hospital of Nanjing Military Command from Feb. 2012 to Feb. 2013 was prospectively collected and reviewed retrospectively. The 5 robotic splenectomies were performed by the same surgeon consecutively. Evaluation was focused on operation procedure, postoperative complication and recovery. Results:All of the patients were operated successfully with no conversions to open surgery or postoperative complications, and they all recovered well. The mean operative time was ( 145 ±50 ) min. The mean time of system assembly was ( 10.5 ±5.2) min. The mean blood loss was (45 ± 12) ml. The average postoperative hospital stay was (3.3±2.5) d. Conclnsions:These results illustrate the safety, feasibility, maneuverability and practicability of robot-assisted splenectomy. It possesses many advantages such as less trauma, less blood loss, less postoperative pain and shorter hospital stay.
