慢性蛋白尿患者血清ANCA测定在SLE诊断中的价值

目的 探讨慢性蛋白尿患者血清ANCA测定在SLE诊断价值.方法 对SLE慢性蛋白尿患者40例（分为ANCA阳性组和ANCA阴性组）和非结缔组织病30例慢性蛋白尿患者,采用间接免疫荧光检测和ELISA检测其血清ANCA及尿常规检测.所获数据采用x 2检验.结果 原发性小血管炎组ANCA阳性率55.56％.SLE患者ANCA阳性检率57.50％.对照组均未检出.SLE患者ANCA检测阳性与阴性组尿常规比较P＜0.05,有显著性差异.SLE患者ANCA阳性与阴性组抗核抗体、抗ds-DNA抗体、血沉与补体比较,P＜0.05,有显著性差异.结论 ANCA阳性程度与SLE的进展和复杂程度呈正相关,慢性蛋白尿患者测定血清ANCA对诊断和鉴别系统性血管炎有其参考价值.

Determination of serum ANCA in the SLE patients with chronic proteinuria diagnostic value in diagnosis

Objective to explore the determination of serum ANCA in the SLE patients with chronic proteinuria di- agnostic value. Methods 40 patients with SLE chronic proteinuria （divided into A NCA positive group and A NCA nega- tive group） and 30 cases of connective tissue disease, patients with chronic proteinuria by indirect immunofluorescence test and ELISA to detect the serum ANCA and routine urine test. The data obtained by X 2 test. Results primary small vasculitis group ANCA positive rate was 55.56%. SLE patients with ANCA positive rate was 57.50%. The control group were not checked out. SLE patients with ANCA routine urine test positive and negative group comparison （P 〈 0.05, there is significant difference. S L E patients with A N CA positive and negative groups antinuclear antibodies, ds - DNA antibod- ies, blood sedimentation and complement the comparison, P〈 0.05, there is significant difference. Conclusion ANCA positive degree was positively correlated with the progress of SLE and complexity, patients with chronic proteinuria serum ANCA for diagnosis and identification of systemic vasculitis is reference value.
