经脐入路腹腔镜胆囊切除术及脐部切口整形的技巧

目的：探讨经脐入路腹腔镜胆囊切除术（laparoscopic cholecystectomy, LC）及脐部切口整形的技巧。方法：总结分析2009年4月至2013年3月为610例胆囊结石患者施行经脐入路LC及脐部切口整形技巧的临床资料。结果：607例成功完成经脐入路LC，3例因腹腔严重粘连，术野暴露极困难中转三孔法LC。脐部切口整形效果满意，切口愈合良好，达到了腹壁无瘢痕效果。手术时间7～60min，多数约18min完成手术，无中转开腹。术后3—5d出院，随访1～48个月，效果良好。结论：经脐入路LC更加微创，腹壁美容效果更好，但操作难度较大，术者应具备熟练的三孔法LC的操作技巧。

The transumbilical laparoscopic cholecystectomy and the techniques of umbilical incision plastic

Objective：To investigate the techniques of transumbilical laparoseopie cholecysteetomy （ LC ） and umbilical inci- sion plastic. Methods：The clinical data of 610 patients with gallstones who underwent transumbilical LC and umbilical incision plastic from Apr. 2009 to Mar. 2013 were summarized and analyzed. Results： Among 610 patients with gallstones,607 cases of transumbilieal LC were successful. 3 patients were converted to three-port LC because of severe abdominal adhesions and extremely difficult surgical field exposure. Umbilical incision plastic was satisfied without scar. The operation time was 7-60 min with 18 min on average. The hospi- talization time Was 3-5 d. Prognosis was good with a follow-up of 1-48 months. Conclusions：The technique of transumbilical LC is difficult. However,it＇s not difficult for surgeons who have mastered the three-hole LC. This technique is better with more minimal invasion and better cosmetic results.
