彩色多普勒超声检测糖尿病患者颈动脉的临床意义

目的：应用彩色多普勒超声对老年糖尿病患者的颈部动脉进行监测，以探讨糖尿病与颈动脉硬化的关系及颈动脉硬化与心、脑血管疾病的关系。方法：对临床确诊的糖尿病患者72例与同龄对照组50例进行彩色多普勒超声监测，测量双侧颈总动脉、颈内动脉、颈外动脉的内径、内膜一中层厚度(IMT)、管壁有无斑块以及斑块的大小、斑块的性质，动脉管腔内的血流性状及血流速度等。结果：糖尿病组与对熙组比较，颈动脉内壁明显粗糙，IMT增加，粥样硬化斑块发生率增高。其中12例合并脑梗塞、7例合并心肌梗死患者IMT均增厚，粥样硬化斑块的形成更为多见。结论：彩色多普勒超声能清晰显示糖尿病患者颈动脉病变情况．糖尿病患者颈动脉硬化明显高于对照组．并且颈动脉硬化与心脑血管疾病有密切关系。

The clinical meaning of checking the patients with diabetes by color dopplor ultrasoundraphy.

Objective:To study association between diabetes and carotid atherosclerosis and its correlating with heart,brain and blood vessel disease by color Doppler in senile patients with diabrtes. Methods:72 patients with diabetes and 50 patients contra matched for the same age, Using Color Doppler for checking the diameter of carotid and intima-media and thickness(IMT), Blood flow characteristics and velocity of Common Carotid, internal and externat carotid artery and detecting weather to has palque in the wall of carotid arteries and the size and characteristics of plaque. Results: Compared with contra group, in diabetes group, the inner - wall of the carotid arteries were more rough, IMT and atheromatous plaque increase 7 cases with myocardial infarction were more obviously searched to have thicker IMT and sclerosed plague. Conclusions:For patients with diabetes,Color Doppler has important clinical value for diagnosing the carotid atherosclerosis, carotid atherosclerosis is more clearly observed and closely relationship with heart brain and vessel diseases.
