局麻下腹腔镜全腹膜外疝修补术28例报告

目的:探讨局麻下腹腔镜全腹膜外疝修补术(total extraperitoneal prosthesis,TEP)的可行性和临床应用特点。方法:回顾分析我院在局麻下为28例患者施行TEP的临床资料。其中腹股沟斜疝20例,腹股沟直疝8例。结果:手术均在局麻下完成,26例顺利完成手术,2例中转开放手术。手术时间45~85min,平均56min,术后住院1~5d,平均3d,术后无阴囊血肿、补片感染、疼痛等并发症发生,随访1~20个月,无复发病例。结论:恰当掌握手术适应证,局麻下TEP是可行的,更能体现微创的优势。

Laparoscopic total extraperitoneal prosthesis under local anesthesia:with a report of 28 cases

Objective:To explore the feasibility and clinical application of laparoscopic total extraperitoneal prosthesis(TEP) under local anesthesia.Methods:The clinical data of 28 patients underwent TEP under local anesthesia were retrospectively analyzed.Among these cases,there were 20 indirect inguinal hernia and 8 direct inguinal hernia.Results:All operations were performed under local anesthesia,among which 26 cases were successfully performed,2 cases were converted to open surgery.The operating time was 45 to 85min(mean 56min),the hospitalization time after operation was 1 to 5d(mean 3d).There were no complications of scrotum hematoma,patch infection or pain after operations.Follow-up ranged from 1 to 20 months,there was no recurrence.Conclusions:TEP is feasible under local anesthesia,and it has the advantages of minimal invasion under suitable indication.
