发作期哮喘患者T细胞亚群的变化及其与肺功能的关系

摘    要:目的 :探讨发作期哮喘患者外周血T细胞亚群的变化及与肺功能的关系 .方法 :采用直接免疫荧光法 ,应用流式细胞仪 ,检测 34例急性发作期哮喘患者及 2 1例健康人外周血CD+ 3 T ,CD+ 4 T ,CD+ 8T细胞的百分率及CD+ 4 /CD+ 8的比值 ,同时测定 2 4例哮喘发作期患者FVC ,FEV1,FEV1/FVC ,FEF 2 5 %~ 75 % .结果 :发作期哮喘患者外周血CD+ 4 T显著高于对照组 (P <0 0 5 ) ;CD+ 8T细胞著性低于对照组 (P <0 0 1) ;CD+ 4 T细胞与CD+ 8T细胞的比值显著高于对照组 (P <0 0 1) ;CD+ 3 T细胞与对照组比较差异无显著性 (P >0 0 5 ) .FVC与CD+ 3 T水平呈负相关 (r =- 0 4 85 ,P <0 0 5 ) ,与CD+ 4 T水平呈负相关 (r =- 0 4 17,P <0 0 5 ) ;FEV1与CD+ 4 T水平呈负相关 (r =- 0 5 12 ,P <0 0 5 ) ,与CD4/CD8比值呈负相关 (r =- 0 5 30 ,P<0 0 1) ;FEF 2 5 %~ 75 %与CD4/CD8比值呈负相关 (r =- 0 5 0 1,P <0 0 5 ) ;FEV1/FVC与CD+ 3 T ,CD+ 4 T ,CD+ 8T及CD4/CD8比值无相关关系 (P >0 0 5 ) .结论 :急性发作期哮喘患者T细胞亚群的变化表明T细胞免疫功能紊乱在哮喘变应性气道炎症中起重要作用 ,且哮喘患者T细胞免疫功能异常与某些呼气流速的肺功能指标有负的相关性

Abstract:Objective: To investigate the level changes of T lymphocyte subsets in peripheral blood in patients with acute asthma attacks and their relationship with pulmonary function. Methods: Levels of T lymphocyte subsets in peripheral blood in 34 patients with acute asthma attacks and 21 healthy subjects were measured by flow cytometry. Pulmonary function was measured in 24 patients with acute asthma attacks. Results: The level of CD + 4T in patients with acute asthma attacks was significantly higher than that in normal subjects (P<0.05). The level of CD + 8T was significantly lower than that in normal subjects (P<0.01); CD + 4/CD + 8 was also significantly higher than that in normal subjects (P<0.01). There was no significant difference in the level of CD3+t between patients with acute asthma attacks and healthy subjects (P>0.05). The levels of FVC and CD + 3T were negatively related (r=-0.485, P<0.05). The levels of FVC and CD + 4T were negatively related (r=-0.417,P<0.05).Conclusion: The level changes of T lymphocyte subsets in peripheral blood play an important role in patients with acute asthma attacks, T lymphocyte subsets and pulmonary function were negatively related in some degree.
