准分子激光血管成形术的动物实验和临床应用

摘    要:为实现利用国产激光器施行血管成形术的愿望,利用308nmXeCl准分子激光器消融活体兔髂动脉粥样硬化斑块及活体狗股动脉血栓。兔狭窄病变动脉15条,成功率93%;闭塞动脉19条,成功率89%;狗血栓动脉20条,成功率85%。1992年用同类激光器治愈一例老年妇女的髂动脉上段阻塞。证实用国产器件配合紫外光纤耦合系统进行准分子激光血管成形术的临床应用是可行的。

Abstract:Excimer laser angioplasty has been widely used in clinical practice abroad. Although a few clinical studies were reported at home, they were based on laser equipment and technology introduced from abroad. Therefore the cost is high and its practical application is limited. For this reason, we used a China made XeCl laser to study excimer laser angioplasty on animals. Experimental atherosclerotic stenosis and occlusion were induced in rabbits and femoral arterial thrombosis was induced in dogs. The laser energy was delivered into the diseased arteries via an UV optical fibre to ablate the lesions. The well recovered blood flow was obtained in 14 out of 15 atherosclerotic stenosis in rabbits, 17 out of 19 complete occlusions in rabbits, and 17 out of 20 femoral arterial thromboses in dogs. Based on the successful animal experiment, we performed an operation for a 84 year old woman, who was suffering from a stenosis of the left iliac artery, using our China made excimer laser system in December 1992. The 2.5 cm long stenotic lesion of the artery was ablated and the blood circulation was restored immediately. It was demonstrated that both our excimer laser apparatus and the UV optic fiber coupling system are well feasible for clinical laser angioplasty.
