脑梗死患者的TCD分析及研究

摘    要:目的 :观察脑梗死的TCD表现以及TCD对脑梗死诊断的意义。方法 :将 110例脑梗死患者的TCD结果进行分析研究。结果 :110例脑梗死患者中TCD异常率 93.6 3% ,大面积梗死组异常率 98.4 1% ,小面积梗死异常率 87.2 3% ,两组比较有显著性差异 ,P <0 .0 1。主要异常表现有狭窄、闭塞、血流减慢、血管痉挛、脑动脉硬化。TCD与影像学检查定位符合率为 80 .4 9%。两者比较无显著性差异 (P >0 .0 5 )。 10 3例异常TCD结果中 32例有侧枝循环形成。结论 :TCD检测脑梗死的异常率与梗死的面积有明显的关系 ,梗死面积越大异常率越高 ,且能结合临床对脑梗死作出病因和定位诊断 ,另外通过对侧支循环的判断为预测脑梗死预后提供了重要依据。因此TCD不失为诊断脑梗死的重要手段之一。

Abstract:Objective: To observe TCD change in cerehroinfarct and its diagnostic significance.Methods:The change of cerebroinfarct's TCD and it's diagnosis significance were analysed.Results:Abnormal percentage of cerebroinfarct in 110 cases was 93.63%,great area infarcts was 98.41%,small area infarcts was 87.23%,and there was an obvious significance between them( P <0.01).The abnormal changes were stricture,obliteration,slow bloodflow,vascular spasm, and arteriosclerosis.The TCD change accordant percentage of the image's location was 80.49%( P >0.05).There were abnormal TCD change in 103 cases, and 32 cases of them had formation of collateral circulation. Conclusion: We considered that there is an obvious relation between abnormal change of TCD in cerebroinfarct and it's infarct area.The greater infarct area ,the higher in abnormal percentage,which might help to decide the aetiology of infarct location diagnosis.The formation of collateral circalation could provide important basis in prognosis after infarct.TCD can yet be regarded as an important method to diagnose cerebro infarct.
