食管上皮癌变过程中p27和增殖细胞核抗原的表达及其意义

目的　探讨 p2 7和增殖细胞核抗原 (PCNA)在食管鳞癌发生发展过程中的表达及意义。方法　正常食管粘膜、非典型增生及浸润癌各 6 4例 ,以免疫组化S P法检测p2 7和PCNA的表达状态。结果　p2 7在正常食管粘膜表达率最高 (90 .6 % ) ,随食管粘膜上皮病变的发展表达率呈下降趋势 ,且p2 7与食管癌组织学分级和淋巴结转移有关 (P <0 .0 5 ) ;而PCNA在正常食管粘膜表达率最低 (35 .7% ) ,PCNA的表达与浸润深度和淋巴结转移有关 (P <0 .0 5 ) ;p2 7与PCNA在非典型增生、浸润癌中的表达与正常组比较均有显著差异 (P <0 .0 1) ;p2 7与PCNA在浸润癌中的表达呈负相关。结论　p2 7和PCNA的异常表达在食管鳞癌发生发展中起重要作用 ,并可作为判断食管癌恶性程度、预测淋巴结转移趋势、指导制定合理治疗方案和预后评估的指标。

Expressions and significance of p27 and PCNA in esophageal squamous cell carcinogenesis

Objective To investigate the expression and significance of p27 and proliferating cell nuclear antigen (PCNA) in esophageal squamous cell carcinogenesis.Method Immunohistochemical (S-P) method was used to detect the expressions of p27 and PCNA in the processes of normal esophageal mucosa epithelium,dysplasia and invasive carcinoma respectively.Results The expression rate of p27 was the highest in normal esophageal mucosa epithelium and there was a descending tendency with the progression of esophageal mucosa lesions.Meanwhile,the expression of p27 correlated with the grading and metastasis of lymph nodes( P <0.05).The expression rate of PCNA was the lowest in normal esophageal mucosa epithelium ,and have relation with the metastasis of lymph nodes( P <0.05); Compared with the normal esophageal mucosa epithelium,the expressions of p27 and PCNA in dysplasia and invasive carcinoma there was a significant difference( P <0.01).The expresson of p27 has negative correlation with PCNA expression.Conclusion The abnormal expressions of p27 and PCNA played an important role in esophageal mucosa carciongenesis,and they Would be of significance in inflecting the malignant degree,indicating lymph node metastasis,guiding treatment and evaluating tumor prognosis.
