基底神经节与皮层脑损害致汉语失写症对比研究

摘    要:目的研究基底神经节与皮层损害致汉语失写症的异同。方法对41例基底神经节损害及21例病变累及左侧大脑皮层区患者,采用ABC法及CAB法测试其口语和书写能力,统计言语障碍类型,计算各项书写得分和失写指数,比较基底神经节与皮层之间的差异。结果24例基底神经节损害失写患者与21例皮层损害失写患者在自动书写(86.58%,51.64%)、听写(71.03%,33.48%)、看图书写(54.62%,22.3%)及主动书写(50.87%,18.57%)的失写指数差异有显著性(P<0.01);在句法、完全性失写率及简单词汇记忆方面差异有显著性(P<0.05)。结论基底神经节损害导致的失写与皮层损害导致的失写有各自特点,两者相互之间部分性质存在交叉重叠现象。基底神经节作为皮层下结构,对皮层参与的书写句法形成起调控作用,两者间存在相互作用。

Abstract:ObjectiveTo study the difference in Chinese agraphia between basal ganglia and cortex damaged patients. MethodsABC and CAB were used to examine the ability of oral language and writing of 41 patients with basal ganglia damaged and 21 patients with cortex damaged. The different types of aphasia and agraphia were detected. The agraphia quotient (AgQ) and writing scores were calculated. Comparing difference with the basal ganglia and cortex. ResultsThere were significant difference in automatic writing(86.58%,51.64%), dictation(71.03%,33.48%), picture writing(54.62%,22.3%) and spontaneous writing(50.87%,18.57%) between 24 cases basal ganglia and 21 cases cortex damaged (P<0.01). What's more, the difference was significant in syntax, the rate of the global agraphia and simple words memory too (P<0.05). ConclusionThe characters of agraphia caused by basal ganglia and cortex damage are similar in some aspects and dissimilar in other aspects. As the subcortex structure, the basal ganglia can modulate the syntax during the scentence processes which the cortex took part in.There was interactive between them.
