阴茎环形加腹侧浸润麻醉在包皮环切中的应用

目的比较阴茎背神经阻滞加阴茎环形浸润麻醉与阴茎环形加腹侧浸润麻醉在商环法包皮环切术中的应用及麻醉效果。方法选取包皮过长的成人患者68例,随机分成2组,对照组和观察组,每组34例,分别在1%盐酸利多卡因行阴茎背神经阻滞加阴茎环形浸润麻醉及阴茎环形加腹侧浸润麻醉下行商环法包皮环切术,记录麻醉达成时间、术后即时及24h后疼痛评分、追加麻醉药物人数。结果平均麻醉达成时间观察组为(3.2±1.1)min,对照组为(5.1±1.7)min;术后疼痛评分观察组为(2.9±1.7)分,对照组为(6.4±1.9)分;术中追加麻醉药物观察组为4例(11.76%),对照组为13例(38.23%);两组间差异有统计学意义。术后24h疼痛评分两组间差异无统计学意义。结论阴茎环形加腹侧浸润麻醉下行包皮环切术,具有手术时间缩短,术后疼痛轻,术中追加麻醉药物少特点,值得临床推广应用。

The Application of Circular Plus Ventral Upward Infiltration for Circumcision

Objective To investigate the clinical application and effect of circular plus dorsal penile infiltration and circular plus ventral upward infiltration for adult male circumcision with Shang Ring.M ethods 68 cases of prepuce and phimosis were treated with circumcision cerclage.Patients were randomized into two distinctive groups.observed group,circular plus dorsal penile infil tration;and control group,circular plus ventral upward infiltration.Record anesthesia reached in time,pain scores in real-time,and 24 hours after surgery and the number of additional narcotic.Results In total,68 adult uncircumcised men were included in to the study.The average anesthesia time was（3.2±1.1） min in observed group,（5.1±1.7） min in control group;pain scores in realtime after surgery was（2.9±1.7） fen in observed group,（6.4±1.9） fen in control group;there are 4（11.76%） men need additional anesthesia drugs in operation in observed group,13（38.23%）men in control group and it were significant.C onclusion Circular plus ventral upward anesthesia is an easy and feasible technique for local anesthesia in？circumcision,which shortened anesthesia time,post operative pain,less additional narcotic,worthy of clinical application.
