甲状腺吸131碘率检查对亚急性甲状腺炎的早期诊断价值

目的：探讨甲状腺吸131碘(I)率检查对亚急性甲状腺炎(简称亚甲炎)的早期诊断价值。方法：用放射免疫分析法(RIA)对235例亚甲炎患者进行血清T3、T4、促甲状腺激素(TSH)、甲状腺球蛋白抗体、过氧化物酶抗体检测，同时进行甲状腺吸131I率测定，并与健康对照组和甲亢组(Graves病)患者进行对比分析。结果：亚甲炎组血清T3和T4值均高于正常对照组(P＜0.01)，与甲亢组差异均无显著性(P＞0.05)；甲亢组吸131I率2h、6h、24h均高于对照组(P＜0.01)，而亚甲炎组2h、6h、24h却明显低于对照组和甲亢组(P＜0.01)，与血清T3和T4升高呈分离现象。结论：血清T3和T4值升高而甲状腺吸131I率降低呈分离现象是亚甲炎诊断的特异性指标。

Value of thyroid 131I uptake test in early di agnosis of subacute thyroiditis

Objective:To assess the value of nuclear medical examination in early diagnosis of subacute thyroiditis.Methods:Serum T 3,T 4,TSH and TGAb,TPoAb were measured by radioimmnoassay(RIA) in 235 patients with early phases of subacute thyroiditis and compared with 30 patients with Graves disease,and control normal subjects,meanwhile 131 I uptake rate of thyroid was determined.Results:The serum T 3,T 4 value of subacute thyroiditis was higher than that of control group( P <0.01).There was no significant difference as compared with the Graves' group( P >0.05).The 131 I uptake rate of 2 h,6 h,24 h in Graves' group was higher than that in normal subjects( P <0.01),while the rate of 131 I uptake of 2 h,6 h,24 h in subacute thyroiditis group was significantly lower than that in normal control group or Graves' group( P <0.01).Conclusions:It is a special diagnostic index of early subacute thyroiditis that T 3,T 4 are higher and 131 I uptake rate is lower to assume a separate phenomenon.
