椎间盘镜下建立椎弓根钉通道行后路腰椎椎体间融合术的研究

摘    要:目的探讨椎间盘镜下建立椎弓根螺钉通道行后路腰椎椎体间融合术的临床疗效。方法应用椎间盘镜下置入椎弓根钉棒系统技术进行后路腰椎椎体间融合(PLIF)术治疗单节段下腰椎病变患者21例。结果手术时间平均180 min,出血量平均220 ml,住院时间平均22 d。21例患者随访2~58个月,视觉疼痛模拟评分(VAS)术前为(7.5±1.8)分,术后降至(2.6±1.4)分,差异有统计学意义(P〈0.05);Oswestry功能指数术前为(46.2±3.8)%,术后降至(23.7±2.1)%,差异有统计学意义(P〈0.05);Nakai分级优良率95.2%。结论微创PLIF术具有切口小、术中出血少、软组织损伤小、术后恢复快、多裂肌萎缩和腰背痛发生率低等优点。

Abstract:Objective To explore the clinical efficacy of posterior lumbar interbody fusion by the establishment of pedicle screw channel under diskoscope.Methods 21 patients with single lumbar segment change accepted interbody fusion disc endoscopic placement of pedicle screw-rod system by minimally invasive technique PLIF surgery.Results The average operative time was 180 minutes,the average amount of bleeding was 220 ml,the average hospital stay was 22 days.All patients were followed up 2-58 months.The VAS score in patients with low back pain from the preoperative(7.5±1.8)points reduced to postoperative(2.6±1.4)points.Oswestry functional index from preoperative(46.2±3.8)% reduce to postoperative(23.7±2.1)%,Nakai graded excellent and good rate was 92%.Conclusion Compared with the traditional PLIF,minimally invasive PLIF technique with advantages are small incision,less blood loss,soft tissue injury is small,rapid postoperative recovery,multifidus muscle atrophy and low incidence of low back pain.
