肝叶切除术治疗肝内胆管结石临床分析

目的探讨肝叶切除术治疗肝内胆管结石的疗效。方法回顾性分析340例肝内胆管结石病例采用肝叶切除治疗术式和疗效。结果行左外叶切除197例，占58．0％；左肝叶切除68例，占20．0％；肝方叶切除20例，占5．9％；右前叶切除18例，占5．3％；右后叶切除10例，占2．9％；右后叶 左外叶切除14例，占4．1％；右半肝切除10例，占2．9％；右前叶 方叶切除3例，占0．9％。术后并发症发生率为19．7％。随访1-15年，随访率83．2％，优良率91．9％。结论肝叶切除术是治疗肝内胆管结石的有效手段。

Clinical Analysis of Hepatic Lobectomy for Treatment of Hepatolithiasis

Objective To investigate the efficacy of hepatic lobectomy for hepatolithiasis. Method The efficacy of hepatic lobectomy treatment of 340 patients suffering from hepatolithiasis were analyzed retrospectively. Results Hepatic lobectomy was performed, with 197 cases of left lateral lobectomy (58.0%) , 68 cases of left lobectomy (20.0% ) , 20 cases of quadrate lobectomy (5.9% ) , 18 cases of right anterior lobectomy (5.3% ) , 10 cases of right posterior lobectomy (2.9%), 14 cases of right posterior plus left lateral lobectomy (4.1%), 10 cases of right hemi-hepatomy (2.9%), and 3 cases of right anterior plus quadrate lobectomy (0.9%). Postoperative complication rate was 19.7%. All patients were followed up for one to 15 years(83.2% of follow-up rate). Cure rate was 91.9%. Conclusion Hepatic lobectomy is an effective treatment for hepatolithiasis.
