序贯化营养支持对重症急性胰腺炎并发症的影响

目的探讨序贯化营养支持对重症急性胰腺炎并发症的影响。方法43例重症急性胰腺炎病人根据营养支持的方式不同分为两组,全胃肠外营养支持(TPN)组21例及序贯化营养支持(序贯化)组22例,比较并分析两组的并发症。结果序贯化组总并发症、导管并发症、二重感染、肝功能损害及腹腔感染发生率,均明显低于TPN组(P<0.05);序贯化组恢复经口进食时间及住院时间显著短于TPN组(P<0.05)。结论序贯化阶段性营养支持可以改善重症急性胰腺炎病人的营养状况,是治疗重症急性胰腺炎理想的营养支持方式。

Changing of the complication of severe acute pancreatitis resulted from the sequential individualized stage nutritional support

[Objective] To explore the influence of the sequential individualized stage nutritional support on the complication of severe acute pancreatitis (SAP). [Methods] 43 patients with SAP between June 2000 and January 2005 were divided into two groups according to the different way of nutritional support. Total parenteral nutritional support group (TPN-group, 21 cases) and sequential individualized stage nutritional support group (Sequential group, 22 cases). The complications were analyzed between the two groups. [Results] The incidences of total complications, incubation complication, superinfection, hepatic function injury and abdominal cavity infection in Sequential group were significantly lower than those in TPN-group (P <0.05). Durations of restoring oral in-take and hospitalization in sequential group were obviously shorter than those in TPN-group (P <0.05). [Conclusion] Sequential individualized stage nutritional support, which have the advantages of less complication and shorter hospitalization time, can improve the nutritional status of the patients with SAP. It is a more reasonable nutritional support method to the patient with SAP.
