动脉内血压脉动指数与冠状动脉病变程度的相关性研究

目的:研究动脉内血压脉动指数与冠状动脉病变程度的相关性,探讨影响血压脉动指数的因素。方法:选择215例拟诊冠心病的患者,经右桡动脉行冠脉造影检查,计算Gensini积分;同步记录升主动脉、肱动脉及桡动脉血压参数,并计算相关脉动指数,结合年龄、血糖、血脂、体重指数及超声心动图等参数,研究动脉内血压脉动指数与冠脉病变程度的相关性;并分析影响动脉内血压脉动指数的因素。结果:单因素相关分析发现:年龄、吸烟、总胆固醇、低密度脂蛋白、中心脉动指数、中心脉压、肱动脉脉压、肱动脉脉动指数、射血分数均与Gensini积分相关(P<0.05),动脉内血压脉动指数主要与年龄和女性呈正相关(均P<0.01)。多因素Logistic回归分析显示:年龄(β=0.414,P=0.01)和肱动脉脉动指数(β=0.287,P=0.04)对冠心病有独立的预测价值。结论:动脉内肱动脉血压脉动指数与冠脉病变的严重程度相关,是冠心病的预测因子。

Relationship between intra-arterial blood pressure pulsatility index and extent of coronary arterial atherosclerosis

Objective: To study the relationship between intra-arterial blood pressure pulsatility index and extent of coronary arterial atherosclerosis and investigate the possible affect factors of pulsatility index.Method: Two hundred fifteen patients suspected with coronary artery disease were recruited and coronary angiography from radial artery were done.All the patients′s Gensini score and pressure for ascending aorta artery,brachial artery and radial artery were recorded.Pulsatility index were calculated for each part.Combined age,glucose,lipid,BMI,echocardiography parameters with pulsatility index related,the correlation of pulsatility index with coronary atherosclerosis and the affect factors for pulsatility index were investigated.Results: One-way ANOVA analysis showed that age,smoking,lipid,LDL,central pulsatility index,center pulse pressure,brachial artery pulse pressure and brachial pulsatility index were significantly related with Gensini score(P<0.05).One-way ANOVA found arterial blood pressure pulsatility index was positively correlated with age and female(P<0.01).Stepwise regression logistical analysis for multivariate factors showed that age(β=0.414,P=0.01)and brachial pulsatility index(β=0.287,P=0.04) were independently correlated with coronary heart disease.Conclusions: Intra-arterial blood pressure pulsatility index is correlated with coronary atherosclerosis and it is the predictor for coronary heart disease.
