输血对肝硬化脾切除患者免疫功能的影响

目的研究输血对肝硬化脾切除患者免疫功能的影响。方法测定肝炎肝硬化病人行脾切除术前后外周血CD3、CD4、CD8、NK细胞,并与正常人及术中输血和非输血者进行比较,分析其变化规律。结果32例肝硬化脾切除患者术前CD3、CD4、NK细胞下降,与对照组比较,其差异均有显著性意义(P<0.05)。输血组输血后CD3、CD4、NK细胞下降,CD8升高,与术前比较,其差异亦均有显著性意义(P<0.05)。非输血组术后CD3、CD4、CD8、NK细胞与术前比较,其差异均无显著性意义(P>0.05)。结论肝硬化脾切除患者的细胞免疫功能普遍低下,术中输血会使患者的免疫功能进一步降低,对肝硬化脾切除患者术后近期免疫功能产生抑制效应。

Effects of blood transfusion on cellular immuno- function in patients underwent splenectomy for liver cirrhosis

Objective To explore the influence of blood transfusion on cellular immuno-function in patients underwent splenectomy for liver cirrhosis. Methods Measure T cell subgroup,NK cell in 32 patients underwent splenectomy for liver cirrhosis pre-operation and 1 week post-operation.Patients were divided into transfusion group and non-transfusion group.Comparison was conducted between the 2 groups.Results Comparsion with normal population,decreasing of CD3,CD4,NK cell in the 32 patients pre-operation was statistically significant (P<0.05). In the transfusion group,decreasing of CD3,CD4,NK cell and increasing of CD8 was statistically significant post-operation was statically insignificant(P>0.05). Conclusion While cellular immuno-function is generally low in patients underwent splenectomy for liver cirrhosis, transfusion will reduce further. It will restrain cellular immuno-function in patients underwent for liver cirrhosis in the near future.
