萍乡地区高危型人乳头瘤病毒分型检测及感染情况分析

摘    要:目的:探讨江西省萍乡地区女性人乳头瘤病毒(HPV)感染现状,为宫颈癌的预防和诊治及HPV疫苗的研发、推广提供依据。方法:采用荧光定量聚合酶链式反应(PCR)对3 797例女性患者的宫颈脱落细胞进行HPV基因15种分型检测。结果:①3 797例受检标本中,HPV总阳性率为20.7%,其中单一感染占比79.90%,多重感染占比20.10%;②15种高危型HPV亚型均被检出,按感染率排名前五名依次为HPV52型(6.82%)、HPV58(3.95%)、HPV16(3.79%)、HPV39(8.47%)、HPV18和HPV51(6.75%);③各年龄段中HPV感染情况为:<30岁(21.58%)、30~39岁(18.86%)、40~49岁(18.54%)、50~59岁(26.98%)、≥60岁(24.70%),各年龄组间HPV感染阳性率差异有统计学意义(P<0.05),单一感染占比以30~39岁年龄段最高,而多重感染占比在<30岁年龄段最高;④不同就诊途径患者HPV感染阳性率为:门诊(20.77%)、住院(19.36%)和体检(25.53%),各组间差异无统计学意义(P>0.05)。结论:对区域性HPV分型和感染情况进行统计分析,明确地区性HPV亚型分布特点,有利于制定具有针对性的筛查和防治措施,有利于推进适用HPV疫苗的设计和应用,对预防和控制HPV感染和宫颈癌的发生具有重要的指导意义。

Abstract:Objective: By investigating the current infection status of female human papilloma virus(HPV) infection in Pingxiang Jiangxi Province,to provide evidence for the prevention,diagnosis and treatment of cervical cancer and the development and promotion of HPV vaccine. Methods: The cervical exfoliated cells of 3 797 female patients were subjected to 15 types of HPV gene detection by fluorescence quantitative polymerase chain reaction(PCR). Results:①Among the 3 797 specimens tested,the total positive rate of HPV was 20.7%,of which single infection accounted for 79.90%,and multiple infections accounted for 20.10%;②15 high-risk HPV subtypes were detected,ranking by infection rate,the first five are HPV52(6.82%),HPV58(3.95%),HPV16(3.79%),HPV39(8.47%),HPV18 & HPV51(6.75%);③HPV infection status in all ages:<30 years old(21.58%),30-39 years old(18.86%),40-49 years old(18.54%),50-59 years old(26.98%),≥60 years old(24.70 %),the positive rate of HPV infection among different age groups was statistically significant( P <0.01),the proportion of single infection was the highest in the 30-39 age group,and the proportion of multiple infection was the highest in the <30 age group;④The positive rate of HPV infection in patients from different visiting routes was:outpatient(20.77%),hospitalization(19.36%)and physical examination(25.53%).There was no significant difference between the groups( P >0.01). Conclusion: To know the regional HPV subtype distribution characteristics definitely by statistical analysis of regional HPV typing and infection,it is beneficial to the development of targeted screening and prevention measures,to the promotion of the design and application of HPV vaccine,and it has important guiding significance for the prevention and control of HPV infection and the occurrence of cervical cancer.
