湖北省2012—2017年并殖吸虫病流行现状监测

目的 了解湖北省并殖吸虫病的流行现状。方法 选取湖北省兴山县、恩施市、郧西县、保康县、南漳县、随县、竹山县、巴东县8个县（市）作为监测点,开展主动监测。包括采集溪蟹检测囊蚴感染情况,采用酶联免疫吸附试验（ELISA）检测人群血清并殖吸虫特异性抗体IgG和IgM。结果 2012—2017年在8个监测点共调查3 561人,总体阳性率1.42%(52/3 651),男性阳性率1.22%（21/1 716）,女性阳性率1.68%（31/1 845）,差异无统计学意义（χ²=1.287,P>0.05）,其中兴山县4次调查共1 128人,阳性率2.57%（29/1 128）;共调查溪蟹760只,平均重量9.81 g,阳性率25.00%(190/760),感染度7.15个/只,其中兴山县共调查340只,阳性率27.94%（95/340）。结论 湖北省并殖吸虫病的传播流行风险依然存在,应持续开展监测防控和健康教育宣传,以免出现暴发流行。

Survey on epidemic status of paragonimiasis in Hubei, 2012-2017

Objective To understand the current status of paragonimiasis epidemic in Hubei Province. Methods Eight counties (cities) of Hubei province (Xingshan, Enshi, Yunxi, Baokang, Sui, Nanzhang, Zhushan, Badong) were selected as the investigation sites for active surveillance. The crabs were captured and the metacercaria of Paragonimus infection status were detected. Meanwhile, the blood samples were collected from the residents in the surveillance sites and the unique IgG and IgM antibodies against Paragonimus in the sera were detected by ELISA. Results A total of 3 561 residents were investigated in the active surveillance, the total positive rate of the serology test was 1.42%(52/3 651), while the rates of the male and the female were 1.22%(21/1 716) and 1.68%(31/1 845) respectively, with no statistical significance between them (χ²=1.287, P>0.05). 1 128 residents were investigated in Xingshan County in 2012-2016 and the total positive rate of serology test was 2.57%(29/1 128). The average weight of 760 fresh-water crabs captured was 9.81 g, with the positive rate of 25.00%(190/760) and the infective density of 7.15 per positive crab. 340 fresh-water crabs were captured in Xingshan County and the positive rate was 27.94%(95/340). Conclusions The risk of transmission of paragonimiasis is still exist in Hubei Province. Measures on continuous surveillance and health education should be taken to avoid the appearance of the prevalence or outbreak.
