"气管插管鉴定囊"在急诊抢救插管中的应用--与听诊法对导管位置判定的比较

目的 观察“气管插管鉴定囊”在抢救危重病人气管内插管时对导管位置判定的作用，并和传统听诊法做比较。方法 57例重症抢救病人，气管内插管后随机分为两组，Ⅰ组病人(n=30)先用“气管插管鉴定囊”检测，然后改用听诊法检测；Ⅱ组病人(n=27)先用听诊法检测然后换用“气管插管鉴定囊”检测。结果“气管插管鉴定囊”检测所需花费的时间短于听诊法；两法均检测出了全部14例食管内插管病人；在57例气管内插管病人中，“气管插管鉴定囊”正确检测出53例气管内插管(敏感性为92．98％)，听诊法检测出46例气管内插管(敏感性为85．19％)。结论 用“气管插管鉴定囊”检测导管的位置是一种有价值的新方法。

Comparison of tracheal detecting-bulb and stethoscope during intubation in critically ill patients

Objective To compare the efficacy of tracheal detecting-bulb(TDB)and stethoscope in critically ill patients.Methods Fifty-seven patients with cardiac arrest were enrolled.After tracheal intubation and ETT cuff inflation in the emergency department,the patients were allocated randomly to two groups.In group 1(n=30),patients underwent a TDB test and an auscultation test in sequence.In group2(n=27),patients underwent an auscultation test and a TDB test in sequence.Results The TDB test was faster than the auscultation test;The TDB test and auscultation test indicated all fourteen esophageal intubations.In 57 tracheal intubations,the TDB test correctly indicated 53 tracheal intubations(sensitivity,92.98%).The ausculation test identified 46 tracheal intubations(sensitivity,85.19%).Conclusion The TDB is a valuable diagnostic tool for differentiating tracheal from esophageal intubation.
