益气通腑汤对脱机困难的COPD患者的临床研究

目的 :观察益气通腑汤对脱机困难的慢性阻塞性肺病 (COPD)病人的疗效。方法 :5 0例难以脱机COPD病人随机分为实验组 (益气通腑汤治疗组n =2 5 )及对照组 (常规治疗组n =2 5 ) ,两组均使用抗生素、解痉、化痰等治疗 ,实验组加用益气通腑汤治疗。呼吸机采用SIMV +PSV模式 ,观察治疗 12d。结果 :实验组应用益气通腑汤 6d后自主潮气量、最大吸气负压及临床症状即明显改善 ,优于对照组。 6d时脱机成功率两组分别为 4 0 %、2 0 % ,两组相比P >0 .0 5 ;12d时脱机成功率两组分别为 80 %、4 4 % ,两组相比P <0 .0 1;应用益气通腑汤前后血生化指标稳定。结论 :益气通腑汤对难以脱机的COPD病人 ,能有效改善消化道功能 ,提高呼吸动力 ,改善通气功能 ,提高脱机成功率 ,无不良副作用。

Clinical study on Yiqi Tongfu decoction for difficult to weaning patients with COPD from mechanical ventilation

Objective: To observe curative efficacy on difficult weaning patients with COPD form mechanical ventilation with Yiqi Tong fu decoction. Methods: 350 cases difficultly to weaning patients with COPD were randomized into two groups: treatment group (n=25) based on conventional therapy of western medicine added Yiqi Tongfu decoction and the control group (n=25) based on conventional therapy of western medicine both of which were treated with antibiotics, antispasm agents and the agents of disperse phlegm. SIMV PSV pattern was adopted. All patients had been treated for 12 days. Results: Automomic TV, MIP were increased obviously and the clinical symptoms were decreased by various degree in treatment group after six days. The success rate of weaning in treatment group was significantly higher than that of the control group(80% VS 44%, P <0. 01) after useing decoction for 12 days. All biochemical markers were stable before and after the treatment. Conclusion: For difficultly to weaning patients with COPD the Yiqi Tongfu decoction is able to improve the digestion functions, to increase the respiratory function, to improve the ventilation function and to increase the success rate of weaning from mechanical ventilation.
