摘    要:目的 评估Gyrus等离子体双极切割系统(Gyrus-Plasma Kinetic System,PKS)作经典的经尿道前列腺剜除术的效果。方法 应用Gyrus-PKS和电切镜,使用生理盐水作为工作介质和冲洗液,经尿道切除治疗BPH病人(BipolarTURP)75例。手术方法采用“推切”技术将增生的腺瘤“剜除”。结果 75例手术顺利。除1例术后出血、血凝块堵塞导管,再次手术止血并输血300mL外,其余均无输血。全部病人术后均排尿通畅,尿控良好。术中及术后无低血钠发生。结论 应用PKS手术效果好,同时避免了经尿道电切综合征的发生。同单极高频外科系统比较,这种手术在安全性方面有独特的优势。

Abstract:Objective To evaluate the result of classical transurethral resection of prostate for BPH using the Gyrus-Plasma Kinetic System(PKS). Methods In the group of 75 patients suffering BPH transurethral resection of prostate was performed by utilizing Gyrus-Plasma Kinetic System and using normal saline solution as working medium and irrigation liquid. During the procedure, "Pushing resection" technique(back-hand stroke technique) was applied to enucleate and resect the prostatic adenoma. Results All of 75 patients underwent the operation successfully. No transfusion was needed, except one patient had to re-enter operation to suck the blood clot and stop the bleeding and receive blood infusion of 300 mL. All patients dischaged successfully, passing their water comfortably with perfect control. There is no hyponatremia perioperatively in the group. Conclusions The result of applying PKS in the operation is satisfactory. Because of having avoided transurethral resection syndrome (TURS) fully, comparing with the monopolar TURP we found that the bipolar TURP with PKS have some special advantage in security.
