喉癌及癌旁组织中P53蛋白的表达

应用枸橼酸－LSAB－微波免疫组化技术，用P53单克隆抗体检测了30例喉鳞癌、癌旁交界处及癌旁0．5、1．0、1．5、2．0cm处粘膜和4例正常喉粘膜中P53蛋白的表达。结果显示：①P53在喉癌组织中的阳性表达率为63．3％（19／30）；在癌旁交界处、癌旁0．5、1．0、1．5、2．0cm处的阳性表达率分别为60％（18／30）、36．7％（11／30）、10％（3／30）、0．0％（0／30）、0．0％（0／30）；4例正常喉粘膜中无阳性表达，其间差异显著（P＜0．01）。②P53蛋白阳性表达强度以喉癌组织中最强，随着远离肿瘤距离的增加，表达强度逐渐减弱。③P53蛋白的表达与喉癌部位、临床分期及肿瘤分级无关。结果提示：P53基因突变参与了喉癌的发生，P53过度表达是喉癌发生中的早期事件；距肿离0．5cm以内的癌旁组织，应视为危险区域。

Expression of P53 protein in laryngeal carcinoma and mucosa adjacent to carcinoma

With the citric-ISAB-microwave immunohistochemical technique and the monoclonal antibody Do-7,the expression of P53 protein at different sites was studied in 30 cases of laryngeal squamous cell carcinoma(LSCC) and four subjects who died from accident and had normal laryngeal mucosa. The results showed that the positive rates of P53 expression were 63. 3%, 60.0%, 36. 7%, 10. 0%, 0. 0% and 0.0% respectively, in carcinoma,the juncture area and the sites of 0.5, 1 .0, 1 .5 and 2. 0 cm from the tumors. None in the 4 samples of normal laryngeal tissues expressed P53 .There was significant difference among them (P < 0.01 ). With respect to the staining intensity, intensity staining of P53 was present in carcinoma, and staining intensity gradually weakened with a increase of the distance from tumor. No correlation was found between the expression of P53 and the sites and histological grade of tumor, and clinical stages. The results indicate that mutation of P53 gene is involved in the genesis of LSCC and may be an early event in the development of human squamous cell carcinoma of the larynx. The tissue within 0. 5 cm from tumor should be regarded as a dangerous region.
