导管射频消融治疗阵发性室上性心动过速

摘    要:①目的为了探讨不同病变部位阵发性室上性心动过速(PSVT)的射频消融治疗方法及治疗效果。②方法应用射频消融治疗43例PSVT病人,其中房室结双径路11例,房室旁道32例(左侧旁道23例,右侧旁道9例)。③结果11例房室结双径路病人采用下位法消融均获成功;32例房室旁道病人中,31例消融成功(96.9%),另1例因旁道位于心外膜经心内膜消融失败。手术并发症发生率2.3%.随访28d~3.5年无复发。④结论射频消融是治疗PSVT的有效方法。

Abstract:Objective To study the method and effect of radiofrequency current catheter ablation on treating paroxysmal supraventricular tachycardia(PSVT). Method 43 patients with PSVT were treated with radiofrequency current catheter ablation among which 11 belonged to atrioventricular nodal double pathway type (AVNDP) and 32 atrioventricular accessory pathway type(AVAP, 23 accessory pathways located on the left side of the heart and 9 on the right). Result The slow pathways in all the 11 patients of AVNDP type were ablated successfully by using inferior approach(100%). 31 patients out of the 32 AVAP type were permanently abolished(96.6%). One AVAP failed to be ablated from the endocardium because the accessory pathway was under the visceral pericardium. The complication rate of the operation was 2.3%. There were no tachycardia recurred during the follow up period between 28 days and 3.5 years. Conclusion Radiofrequency current catheter ablation was an effective method to treat PSVT.
