腹腔镜膀胱破裂修补术的安全性和可行性

摘    要:目的评价腹腔镜在膀胱破裂修补中的疗效和安全性。 方法前瞻性纳入22例新疆维吾尔自治区人民医院采用单孔腹腔镜手术(laparoendoscopic single-site surgery,LESS)治疗的膀胱破裂修补术患者,LESS采用由Alexis切口牵开器和外科手套组成的国产单端口装置。 结果共计22例,其中男18例,女4例。平均年龄(40.50 ± 11.83)岁。平均体重指数(24.19±2.61)kg/m2。膀胱破裂的原因是创伤(21例)或医源性损伤(1例)。膀胱破裂平均直径(3.41±1.01)cm。所有患者均使用LESS有效完成。平均手术时间(89.05±11.29)min。平均住院时间(2.91±0.53)d。术后没有患者需要自控镇痛、没有患者发生严重并发症。术后(7.68±2.08)d拔除导尿管。 结论采用配备有单端口装置的LESS修复腹膜内膀胱破裂,对于常规腹腔镜手术是一种可行且安全的替代方案。

Abstract:ObjectiveTo evaluate the efficacy and safety of laparoendoscopic in the repair of intraperitoneal bladder rupture. Methods22 patients who underwent laparoendoscopic single-site surgery (LESS) for intraperitoneal bladder rupture in the People′s Hospital of Xinjiang Uygur Autonomous Region were included prospectively in this study.LESS was performed using a domestic single-port device composed of an Alexis wound retractor and a surgical glove.The study on the safety and effectiveness of this method of treatment. ResultsOf the 22 patients, 18 were male, 4 were female. The mean age was (40.50±11.83)years. The mean body mass index was (24.19± 2.61)kg/m2. The cause of rupture was trauma (n=21) or latrogenic injury (n=1). The mean bladder rupture diameter was (3.41±1.01)cm. LESS bladder rupture repair was completed effectively in all patients. The mean operative time was (89.05±11.29)minutes. The mean length of hospital stay was(2.91±0.53)days. Postoperatively, none of the patients required patient-controlled analgesia, and none developed major complications. The urethral Foley catheter was removed(7.68±2.08)days after surgery. ConclusionsLESS repair of intraperitoneal bladder rupture, which involves a domestic single-port device, was a feasible and safe alternative to conventional laparoscopy in this case series.
