摘    要:目的观察重组组织型纤溶酶原激活物(rt—PA)静脉溶栓治疗超早期脑梗死的疗效、安全性及治疗后凝血功能的改变。方法符合入选条件的患者分两组,治疗组进行rt—PA静脉溶栓治疗;对照组给予非溶栓治疗。对比两组用药前、用药后24h、7d、21d时美国国立卫生院神经功能缺损评分(NIHSS)数值。结果治疗组比对照组的NIH—SS分值明显下降,差异显著,无严重并发症发生,凝血功能无显著改变。结论如果严格掌握时间窗及适应证,rt—PA静脉溶栓治疗超早期脑梗死安全,对凝血系统影响轻微,比非溶栓治疗方法有效.

Abstract:Objective Observation on efficacy, safety and changes in blood coagulation of intravenous rt - PA thrombolytic therapy in patients with hyperacute cerebral infarction was carried out. Methods Patients complied with requirement for selection were allocated into two groups, one group( treatment group) with intravenous rt - PA thrombolytic therapy and another group( control group) with non - thrombolytic therapy. Comparison in these two groups was carried out before treatment and after medication for 24 h, 7 d and 21 d with Stroke Scale(NIHSS) neurological scoring. Results The efficacy of patients in treatment group is much better than that of control group, and the NIHSS scores were significantly decreased. There was no serious complications occurred during the therapeutic course. The change in coagulation function was not significant in patients of treatment group. Conclusion rt - PA intravenous thrombolytic therapy in treatment of patients with hyperacute cerebral infarction is safe and effective under strictly mastering the indications with minimal impact on coagulation system, and it is more effective than non - thrombolytic therapy.
