糖尿病肾病与视网膜病变相关性研究

目的分析糖尿病肾病（DN）与糖尿病视网膜（DR）病变的相关性。方法选取2010年1月～2012年1月诊断治疗的2型糖尿病患者190例,回顾性分析其临床资料及实验室检查,按照糖尿病患者尿蛋白排泄量分为DN0组（无蛋白尿组）、DN1组（微量蛋白尿组）和DN2（临床蛋白尿组）;比较不同组别视网膜改变的情况,分析其相关性。结果①DN2组患者年龄、病程、TC水平、UEAR水平等均明显高于DN0及DN1组,差异有统计学意义（P〈0.05）。②DN0组患者23例单纯型DR,2例增殖型DR,总发生率为21.7%;DN1组DR、DR分别为27例和7例,总发生率为54.8%;DN2组DR、DR分别为7例和4例,总发生率为84.6%。组间两两比较发现,DN2总发生率明显高于其他两组,且DN1组高于DN0组,差异均有统计学意义（P〈0.05）。采用Spearman进行相关性分析发现DN与DR具有正相关性（r=0.328,P〈0.05）。结论糖尿病肾病与视网膜改变具有相关性,呈平行发展。

Study of correlation between diabetic nephropathy and retinopathy

Objective To analyze correlation between the diabetic nephropathy（DN） and diabetes retinopathy（DR）.Methods 190 cases with type 2 diabetes from January 2010 to January 2012 were selected and divided into DN0 group（without albuminuria group）,DN1 group（trace albuminuria group） and DN2 group（clinical proteinuria group） by the urinary protein excretion rate of diabetes patients.Retinal changes of different groups were compared and the cor relation was analyzed.Results ① The patients＇ age,level of course,TC,and UEAR of DN2 group were higher than those of DN0 group and DN1 group,the differences were statistically significant（P 0.05）.②The total occurrence rate of the DN0 group was 21.7% with 23 cases of type DR and 2 cases of proliferative DR.The total occurrence rate of the DN1 group was 54.8% with 27 cases of type DR and 7 cases of proliferative DR.And the total occurrence rate of the DN2 group was 84.6% with 7 cases of type DR and 4 cases of proliferative DR.The total occurrence rate of the DN2 group was higher than the other two groups,and the total occurrence rate of DN1 group was higher than DN0 group,the differences were statistically significant（P 0.05）.Positive correlation was found between DN and DR according to Spearman correlation analysis（r = 0.328,P 0.05）.Conclusion Diabetic nephropathy has correlation with retinal change,they have parallel development.
